Comments: Violation of Permit SW-96/01 condition IV.B.1.g. Motiva must inspect all accessible collection pipes in the leak detection system of fly ash pond 2 at least once each year. Motiva failed to inspect the main pipe draining to Manhole 2 during 2010. Furthermore, Motiva did not inspect the main pipe draining to Manhole 1 after cleaning to determine of sediments blocking the camera's path had been removed.

Violation of Permit SW-96/01 condition VII.C.4. Motiva must report the results of annual cleaning and inspection must within 60 days of the event. Although Motiva completed the cleaning and camera inspections on May 27, 2010, Motiva failed to report the results to the Department until September 8, 2010.
